If you experience any technical difficulties while completing the following steps, please contact MD-COLLECT at (877) 253-3629 or

  1. One of the following Internet browsers:
  2.        Internet Explorer 5 or greater

           Netscape 6 or greater

           Mozilla FireFox 2.0 or greater

  3. You will need to fax a copy of a current state medical license from any one physician in your practice to MD-COLLECT. Please fax to: (336) 761-8852. This is required information.
  4. Once your medical license has been received, your practice will be activated and you can submit accounts.

    Until we receive a valid state medical license from your practice, you may enter accounts into a pending list that can be submitted later and you can explore all areas of the MD-COLLECT application website. You will be unable to submit any pending accounts for collections until your account is activated.

  5. General practice contact information, such as name, address, phone number, physician names, et cetera.
  6. A valid credit card to pay for collection of patient accounts submitted.  We accept Visa, MasterCard, Discover and American Express.  To secure all your transactions, we use VeriSign.
  7. Past due patient accounts to submit for collections.
